TIME of birth is IMPORTANT when preparing your birth horoscope and doing the complex chart comparisons used for the matching and ranking process in the STAR MATES program. If you do not already know the exact hour and minute of your BIRTH TIME, please try to learn it from parents or relatives, or from the hospital where you were born, before you use the ENTRY FORM. In most countries of Europe every citizen's birth time is recorded in the public records. A birth time error of even a few minutes may affect your scores and your matches. If not sure at all, enter a ? mark. If you know to within 15 minutes, enter your best guess to the exact minute. Please give LOCAL TIME, as shown on CLOCKS at your place of birth. (If you are an astrologer and KNOW if Standard Time or Daylight Time was in effect, add ST or DT after the Am or Pm, but do NOT adjust the time itself.)

If you submit an Entry Form NOW it will be for the WAITING LIST for STAGE 2, which will include some changes and additional options which cannot be determined at this time. Please understand that there are no promises or guarantees being offerred at this time that your Entry will qualify for the 200 FREE Memberships, but if you submit a FULLY COMPLETED Entry Form-2 now you will be placed on the STAGE 2 Waiting List and all accepted applications will be processed in the order in which they were received. ***

THANK YOU for your interest in being a volunteer Member in STAGE 1 of "The STAR MATES Experiment" to test the accuracy of a new advanced astrological "compatibility matching" program. After you and 199+ others have submitted valid Entry Forms, I will run my astrology program to match you against all 199+ members, then email you a FREE Search Report which lists YOUR "Top 5" compatible matches and their scores on seven rating Factors listed on the Report. You may then submit one CONTACT REQUEST FORM to request the email address of any or all of your 5 best matches (who may be male OR female), and then correspond with them to find out if they prove to be as compatible as Internet FRIENDS as the program's rating system predicted. Perhaps you'll find a cyber-space Soul-Mate! Other Members may have YOU on THEIR "Top 5" lists, and you might receive email from them too!

By submitting this ENTRY FORM, you are indicating your agreement to abide by the terms of "the Experiment" and your acceptance of these TERMS and CONDITIONS page. Please note that "@hotmail.com" or other anonymous email addresses are NOT acceptable for the purposes of "The Experiment". You will be receiving email addresses of people who are giving you their personal or company email address, and are entitled to expect the same from you.
UPDATE JUN 15 1997: you may now submit an anonymous Contact-Address to be given to Members who have you on their Top 5 List and wish to contact you; provided you have registered a confidential Private-Address on your Entry Form which can be checked to verify that you are who you say you are. The Private-Address will not be given out, but will be used to send a Confirmation letter to ensure that you are a sincere adult applicant who will be responsible for his actions in his contacts with other Members. (If that email letter is returned as undeliverable, your application will be assumed to be a prank and will be deleted). STAGE 1 Members who now wish to use an anonymous Contact-Address for any future Contact Requests in STAGE 1 may use the Entry Form-2 to submit the new email address.
Entry Form-2 may also be used to make any updates or corrections; or to make a request to remain active in the ASTRO database for free until 1998 and participate in STAGE 2 of The Experiment. Please be sure to enter your STAGE 1 Member Number in the box which requests it, so I know you are already a Member and will automatically be included in the database when the new Members are being matched to each other and the STAGE 1 Members who indicated a wish to participate. If you do not enter your Member Number, you will be considered a new applicant and may be put on the Waiting List if you apply after the first 200 new applications have been accepted.

Please fill in **EVERY** box on the Entry Form-2 questionnaire and then select the SEND button near the bottom of the page. A form with *ANY* empty boxes cannot be processed, so PLEASE don't waste your time submitting an Entry Form if you do not want to complete it fully. Your last name or birth year will NOT be disclosed, so please do not leave it out if you wish to participate.

Your Answers to questions in RED will appear on Search Reports. Your E-MAIL ADDRESS will be given ONLY to Members who were matched to you, and who provided theirs. ALL OTHER DATA (not in red) will be kept CONFIDENTIAL. I will not release your address to any other mailing lists, and my professional ethics as an astrologer will not allow me to tell your birth year or time to anyone without your permission.

When you use the Entry Form, please enter your APPLICATION NUMBER, which is the same as the VISITOR number at the TOP of the Entry Form page ("You are the ___ VISITOR").
Write this number down, so you can find out the status of your Application listed under that same Application Number on the |CHECK-IN| page in 3 to 5 days. This number will be used so that your name will not appear anywhere on the pages of the website, but YOU will know that any message or information posted beside that Application Number will be for YOU. This is especially important if you made an error in entering your email address, because it will be the only way to let you know when I am unable to write to you without knowing the correct address.

Please visit again in 3 to 5 days after sending your Entry Form, so that you can check the |CHECK-IN| page and see if your Application Number is listed as accepted as complete, or requires re-application because your email address was incorrect or some other information was left out.

To COMPLETE your Entry Form now...

When your application is sent successfully, you will AUTOMATICALLY be sent to the CHECK-IN page, which you should bookmark so you can CHECK IN after 3 to 5 days to see if your Application Number is list as received and accepted. If the Entry Form cannot be SENT, the mail server may be too busy, or be off-line for maintenance. Please WAIT a minute and try the SEND button again (but do NOT select it again UNTIL you see a "Cannot reach remote host" or similar message). Sometimes the Internet is slow, and it takes a minute or so to make the connection. Often you CAN connect on the second attempt.

Your data is sent by E-MAIL, which is not always 100% reliable, so if you do NOT see your Application Number on the |CHECK-LIST| page within 5 days, send a note to starman@astrologyzine.com with the DATE and DETAILS of your application, OR just submit a new Entry Form using your original number.
